We are looking for a passionate and empathetic Support Coordinator to join our team. As a Support Coordinator, you will play a crucial role in helping individuals access the support and services they need to live independently and thrive. You will work closely with clients, their families, and service providers to create and implement individualized support plans, advocate for clients' rights and needs, and ensure that they receive the highest quality of care and services.

**Key Responsibilities**
- Conduct comprehensive assessments of clients' needs and goals.
- Develop and implement individualized support plans in collaboration with clients and their families.
- Advocate for clients to access appropriate services and resources, including healthcare, housing, employment, and social services.
- Coordinate and monitor service delivery, ensuring that services are aligned with clients' goals and preferences.
- Collaborate with a multidisciplinary team, including healthcare professionals, social workers, and community agencies.
- Provide information, guidance, and support to clients and their families to navigate the support system.
- Maintain accurate and up-to-date client records and documentation.
- Monitor and evaluate the effectiveness of support plans and make necessary adjustments.
- Participate in ongoing training and professional development opportunities.

**Qualifications**:

- Bachelor's degree in social work, psychology, counselling, or a related field (preferred).
- Previous experience in support coordination or case management is a plus.
- Strong interpersonal and communication skills.
- Excellent organizational and time management abilities.
- Empathy, compassion, and a genuine desire to help others.
- Knowledge of local resources and support services.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
- Blue Card and Yellow Card
- First Aid and CPR
- NDIS Modules
