In this full-time position reporting directly to the Clinical Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success. **The Care Coordinator will be responsible for**:

- Building strong connections with clients and their families
- Assigning staff to clients based on staff qualifications and availability, as well as client preferences
- Organise support for clients and monitoring and reporting on their progress
- Referring clients to agencies that can provide additional help
- Assessing client needs and resources for health, welfare and other facilities and services
- Maintaining accurate records of incidents and feedback as required
- Re-organising work priorities to meet the needs of a fast-paced environment
- Supporting all clients to meet their goals
- Conducting Client Care Assessments and Care Planning
- Providing clinical guidance to other team members for client management
- Discussing and promoting Right at Homes services with medical professions and referral sources
- Ability to work in a fast-paced, dynamic environment

**Qualifications**:
**Minimum Certificate III/IV in Individual Support/Disability**

**Diploma in Nursing (Enrolled Nurse)**

**Preferred Skills**:

- Familiar with; or previously worked in the aged care sector.
- In-depth understanding of SCHADS Award and Fair Work Requirements are highly desirable.
- Previous experience with Visual Care and/or other rostering systems.
- Understanding and experience with government-funded Home Care Packages
- A willingness to learn on the job
