In this full-time position reporting directly to the Clinical Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success. The Care Coordinator will be responsible for:

  • Building strong connections with clients and their families
  • Assigning staff to clients based on staff qualifications and availability, as well as client preferences
  • Organise support for clients and monitoring and reporting on their progress
  • Referring clients to agencies that can provide additional help
  • Assessing client needs and resources for health, welfare and other facilities and services
  • Responding promptly to emails, enquiries and requests from clients, staff and management
  • Maintaining accurate records of incidents and feedback as required
  • Re-organising work priorities to meet the needs of a fast-paced environment
  • Supporting all clients to meet their goals
  • Conducting Client Care Assessments and Care Planning
  • Providing clinical guidance to other team members for client management
  • Discussing and promoting Right at Homes services with medical professions and referral sources
  • Ability to work in a fast-paced, dynamic environment
Qualifications

Minimum Certificate III/IV in Individual Support/Disability

Diploma in Nursing (Enrolled Nurse)

Preferred Skills
  • Familiar with; or previously worked in the aged care sector.
  • In-depth understanding of SCHADS Award and Fair Work Requirements are highly desirable.
  • Previous experience with Visual Care and/or other rostering systems.
  • Understanding and experience with government-funded Home Care Packages
  • A willingness to learn on the job
