**Position Overview**
We are seeking an experienced Team Leader to join our team. We are specifically looking for someone who can support a team in complex care supports, in their daily living activities, promoting independence and enhancing their quality of life. The supports will be varied, but with a common goal to enhance the ability of the individual to live independently. With the support of a clinical allied health team, you’ll be expected to implement several support strategies that seek to improve the overall day to day for the individual.
Working Monday to Friday, and the full support of a clinical allied health team, this role is critical the everyday runnings of the program - Including but not limited to;
- Supporting the participant directly
- Scheduling and rostering
- Care Team Meetings
- Incident reporting & Progress reports
- Activity planning
This role would suit some one experiened in either managing supported independent living services, large carer teams, or program coordination.
**Key Responsibilities**:
- Provide support to NDIS participants, including assistance with daily living activities.
- Support participants to kick goals in a
- Facilitate social and community engagement activities.
- Implement individual support plans tailored to each client's needs.
- Maintain accurate progress notes.
- Report incidents where necessary
- Collaborate with team members and healthcare professionals to ensure the best outcomes for clients.
**Qualifications**:
- Certificate IV in Disability or equivalent qualification. (or social work, community services)
- NDIS Worker Screening Check (mandatory).
- Working With Children Check (WWC) (mandatory).
- Experience working with individuals diagnosed with Intellectual Disability, ASD, ADHD and OCD
- Strong communication and interpersonal skills.
- Empathy and a genuine desire to support individuals with disabilities.

Pay: $44.00 - $49.00 per hour
Expected hours: 37 per week
Schedule:
- 8 hour shift
- Monday to Friday
- Rotating roster
- Shift work
- Weekend availability
Licence/Certification:
- First Aid Certification (required)
- Working with Children Check (required)
- NDIS Worker Screening Check (required)
- National Police Certificate (required)
- Driver Licence (required)
