**_System and Care Navigator (Fixed term, 0.8-1.0 FTE) _**
- Improve quality of life to support ageing in place.
- Reduce burden of care of increasing client complexity on Housing officers.
- Support holistic health and wellbeing health assessments - right care at the right time.
- Review and define roles and responsibilities of stakeholders.
- Improve safety and reduction in risk of liability for ageing cohort.
- Improve proactive and strategic approach to identifying and managing tenant needs.
- Increase occupancy, housing stock utilisation, and management of effective refurbishment based on needs.
- Lead project evaluation
A unique and exciting opportunity to utilise both your clinical and project management experience Help lead a multi-agency project aiming to improve systems, tenant experience and health and wellbeing outcomes for public housing residents in and around Richmond.
Roles and responsibilities include, but are not limited to:
- Provide expert opinion on opportunities for system improvements.
- Demonstrate an understanding of objectives and expected outcomes of project.
- Demonstrated understanding of clinical and social referrals pathways that support positive client outcomes.
- Use current evidence base for initial needs assessments and health assessments.
- Work collaboratively to ensure relationships with key stakeholders including the Richmond Housing Office, to support service integration and holistic care addressing identified social determinants of health.
- Initiate and maintain positive relationships with local community and other stakeholders to manage risks and promote our great work.
- Provide advice and expert opinion to the Manager and Executive, to support positive outcomes for clients and enhance service delivery and programs.
- Promptly report and respond to complex clinical and service-related situations.
- Convene project governance meetings and prepare reports as requested.
**Key Selection Criteria**
**Essential qualifications, attributes and skills include, but are not limited to**:
- AHPRA or other relevant clinical registration.
- Certificate or higher qualifications in Project Management
- Demonstrated experience in project management.
- Commitment to improving the health of vulnerable and marginalised communities.
- Demonstrated high level interpersonal skills and stakeholder engagement.
- Previous experience working with diverse communities within public housing.
