_We have an exciting opportunity for an RN or registered _**_allied health clinician_**_ to take on the role of _*_Care Coordinator_*_ and join the Health Independence Program team at Richmond. You will provide support to clients in the post discharge period and assist them navigate the service system._
The PAC program has a flexible service delivery model enabling the program to provide a responsive service which includes purchasing a wide range of health and community services and provision of clinical care by program staff when necessary.
**Roles and responsibilities include, but are not limited to**:

- Intake, screening and assessment;
- Liaising with clients, their carers/families, hospital clinicians and other community service providers;
- Developing care plans with clients, engaging /contracting a range of clinical and in home support services;
- Provision of clinical services when required;
- Developing and maintaining strong networks with hospital clinicians, community service providers and neighbouring Health Independence Programs

**Key Selection Criteria**
Essential qualifications, attributes and skills include, but are not limited to:

- RN or registered allied health clinician with significant experience working within the health and/or community services sector
- Experience working with people with complex issues in a health setting (including homelessness, drug & alcohol, mental health, dementia, refugee and migrant communities), and the proven ability to work with people from CALD (culturally and linguistically diverse) backgrounds
- Enthusiastic, highly organised team player with a strong knowledge of the community services available to clients in the recuperative phase
- Demonstrated ability to identify issues, problem solve, be an effective communicator with a wide range of stakeholders and meet deadlines in a fast-paced environment
- Excellent computer skills including MS Office suite
