JOB TITLE Coordinator of Supports

LOCATION As Required by the Business

SUPERVISOR/MANAGER Team Leader

MAIN DUTIES/RESPONSIBILITIES
- Achieving outcomes and Goals according to the needs of the participant
- Achieving at least 85% Billable Hours in the fortnight
- Keeping Good Relationships with Participants and Families
- Ensuring all documentation is complete up to date and signed
- All hours are accounted for in the Support Coordination Book daily
- Reviews Completed at 1 month 6 months and 9 months
- Yearly Reviews and Service Agreements to be started at least 30 days prior
- Provide support to strengthen a participant’s ability
- To design and build their personal supports with an emphasis on linking a broader system of support across a complex services delivery environment.
- Coordination of supports focusing on supporting participants to direct their lives, not just their services e.g. crisis and resilience development
- Assist participants to connect with NDIS supports, negotiate solutions with multiple stakeholders and build their capacity and resilience.
- Assist participants to develop knowledge, experience and connections with their community and broader systems of support.
- Work with the participant to assist them to understand plan funding and its purpose.
- Identify and understand participant confidence and skills assist participant decide what they want from services.
- Develop and design support solutions to meet participant outcomes.
- Assist the participant to identify and consider support options and link the participant to the broader systems of supports.
- Where practical, create a support and action plan to facilitate participant plan implementation.
- Assist to resolve points of crisis and develop capacity and resilience within the participant's network.
- Coach the participant through challenges.
- Assist participant prepare for the review and reporting of their achieved outcomes.
- Manage rosters, administration and intake process

SKILLS & EXPERIENCE

**Qualifications**:

- Cert IV 4 in Disability or Individual Support or Equivalent
- First Aid
- WWCC
- NDIS Worker Check
- Worker Mandatory Orientation Module

Experience
- NDIS / NDIA Knowledge
- Highly Effective interpersonal Skills
- Ability to overcome barriers
- 1 to 2 years’ experience working with people with a disability
- Working with a Client Data Base Management System

- Efficient in Microsoft Applications
- Communication - Collaborate withall stakeholders to keep them informed
- Interpersonal skills

**Job Type**: Part-time
Part-time hours: 20 per week

**Salary**: $31.00 - $36.00 per hour

Schedule:

- Monday to Friday
- On call

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)
- NDIS Worker Orientation Module (preferred)
- NDISWC Number (preferred)

Work Authorisation:

- Australia (preferred)
