**About the role**
We are currently seeking a full time Tailored Support Coordinator to join our Operations team located in Tuggerah. Reporting to the Executive Manager - Social and Environmental Sustainability, this role provides assessment and planning for occupants and supports them to access a range of individualised services to improve their wellbeing and achieve their goals.

**Key Responsibilities include**:

- Coordinating responses and support for occupants experiencing personal crises or risk to their tenancies;
- Positively engaging with occupants to identify their challenges and set realistic goals, using a strengths-based approach;
- Maintaining knowledge and building working relationships with other community services;
- Referring occupants to internal and external services and support networks, monitoring for quality and effectiveness of the referral processes;
- Overseeing the delivery of Community Development programs and services at properties, in collaboration with the branch Community Participation Officer;
- Liaise with the Tenancy Relations team where specialist community development initiatives and responses are required.

**About You**
To be considered for this role, please ensure you have addressed the following essential selection criteria:

- Minimum of 3 years’ experience in Community Welfare or related discipline - ideally a background in working in support case coordination or case management.
- Diploma or Degree qualified in Social Welfare, Social Science or related discipline
- Demonstrated ability to coordinate the provision of effective and individualised support to achieve positive outcomes for occupants
- Experience working in an environment supporting people with complex needs
- Competent in developing case plans & supporting clients to achieve positive employment outcomes
- Excellent communication skills
- High level of negotiation skills and ability to liaise with key stakeholders.
- Experience in behavioural change strategies
- Demonstrated ability to achieve social outcomes
- Ability to use Microsoft Office systems
- A Current Drivers License
