**About the role**

Are you passionate about mental health and improving life outcomes of people living with complex mental health illnesses? if yes, join us in January 2025 in this exciting new role and be part of our new Clinical Care Program. The aim of this program is to provide greater access to mental health services for those people living with mental illness in the Central Coast region.

Mental Health Services in Primary Care will improve service integration and achieve alignment with a person-centered, stepped care approach. The aims will be achieved by engaging a range of mental health professionals to provide a suite of evidence based mental health services to people that would otherwise have little or no access to such services.

As a **Support Worker - Mental Health**, you will work alongside the Specialist Lead and Case Worker to provide coordinated clinical care for people living with complex and enduring mental illness. You will guide the care that will be provided to clients and collaborate with both the Case Worker and the Specialist Lead to liaise with local networks in the community and provide the support required for each client on their mental health journey.

This is a permanent full-time position based on the Central Coast and covers participants who live on the Central Coast. CatholicCare is open to flexible working arrangements that meet the needs of the program.

**Key responsibilities**:

- Liaise closely with family and carers as appropriate.
- Walk alongside the client to provide practical supports to assist in their recovery.
- Support links with GP’s, psychiatrists, and allied health workers.
- Maintain links within the community and build networks with organisations that provide psychosocial support.
- Provide transport and support to clients to attend allied health appointments.
- Maintain accurate, up to date client focused records of all interactions and progress in accordance with CatholicCare policies and guidelines.
- Provide clear information and data to team leader for reporting purpose.
- Participate in fortnightly group supervision.
- Support Specialist Lead in implementing and adapting the program.
- Ensure compliance with incident management, reporting and escalation requirements and that organisational obligations under mandatory reporting, child protection, safeguarding and WHS are met.
- Ensure you have good working knowledge of mental health legislation and practices.

**To be successful in this role, you will have**:

- Extensive experience working in mental health, social services or similar field.
- National Criminal History Check.
- Working with Children Check.
- NSW Drivers Licence and own car.
- Advise CatholicCare Diocese of Broken Bay in writing of any condition that may impact on the ability to perform the responsibilities required of this role, including any change in circumstance that may impact on your Mandatory Compliance checks as listed.

**Desirable**:

- Qualifications or working toward in mental health, social work, nursing or equivalent.
