**Position Overview**:
The NDIS Intake Officer will serve as the primary point of contact for potential clients, their families, and representatives. The role involves implementing a coordinated intake process to identify client support needs, co-designing support plans, developing service quotes and agreements, and collecting client-specific data. The Intake Officer will instil confidence in potential clients and stakeholders, promoting Yahweh Care as their provider of choice.

**Key Responsibilities**:

- Act as the initial point of contact for all potential clients, families, and representatives, providing responsive service to facilitate their transition from inquiry to service delivery.
- Implement a coordinated intake process to assess client support needs, including routine of clients, developing service quotes, and client agreements and alied health reports.
- Collect and maintain accurate client information within a person-centered active support framework.
- Determine clients' eligibility for services and refer them to other relevant services if required.
- Collaborate with clients, families, Local Area Coordinators, and Support Coordinators to facilitate supports.
- Develop support plans consistent with NDIS plans to ensure the achievement of key outcomes and goals, presenting them to clients or their representatives for approval.
- Liaise with management to develop appropriate quotes that meet client needs and organizational requirements.
- Conduct initial support and risk assessments meetings with Registered Nurse and clients or their representatives.
- Ensure effective and professional communication with the Manager and provide monthly service reports.
- Collaborate with internal and external stakeholders to ensure successful facilitation of NDIS plans and support client outcomes.
- Renewals of any documents to be completed.

**Qualifications and Experience**:

- Demonstrated experience in a similar role within the NDIS or disability services sector.
- Strong understanding of the NDIS framework and person-centered active support principles.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and stakeholders.
- Proven ability to develop support plans and service agreements tailored to individual client needs.
- Strong organizational and time management skills, with attention to detail.
- Proficiency in using client management systems and data reporting programs.
- Ability to work independently and as part of a multidisciplinary team.
- Current NSW driver's license and a vehicle with third-party property insurance.

**Desirable**:

- Experience in mental health or carer support organizations.
- Knowledge of contemporary mental health carer issues.

Pay: $75,000.00 - $80,000.00 per year
