**About the Role**

The Case Manager is responsible for coordinating and managing the delivery of support services to our participants. The role will play a pivotal role in coordinating and managing individualised support plans for our participants, ensuring they receive the services they need to thrive. The Case Manager will also assist the operations and initiatives within the Service Delivery department.

**Key Responsibilities**
- Act as a point of contact for client case management and concerns.
- Assist the case worker team and provide support when required.
- Provide support to the Service Delivery Team on complex cases and operations of the team.
- Lead initiatives to improve case management and service delivery quality.
- Develop and manage case plans tailored to individual participant needs.
- Engage actively with participants and stakeholders to enhance service delivery.
- Maintain meticulous records in compliance with NDIS guidelines.
- Advocate for participants' rights and empower them to participate in their care decisions.
- Stay informed on industry developments and participate in professional growth opportunities.
- Uphold our values of integrity, respect, equality, and community commitment in every interaction.

- Proven experience in client-facing roles, preferably in health, social services, or related fields.
- Strong interpersonal and communication skills, capable of nurturing lasting relationships.

- Committed to personal and professional growth within a forward-thinking company.
- Knowledgeable about the NDIS framework and related legislation.
- Participate in a rotating after-hours roster.

Pay: $80,000.00 - $90,000.00 per year
