**Rose Aged Care Service** is a registered, customer-focused aged care provider based in Blacktown, delivering personalised, compassionate, and ethical home care services across the Sydney metropolitan area. As our business continues to grow, we are seeking an experienced and dedicated **Support Coordinator & Home Care Case Manager** to join our team on a **part-time basis**.
**Position Purpose**:
As the **Support Coordinator & Home Care Case Manager**, you will coordinate and manage the delivery of high-quality, person-centred home care services while supporting clients and their families. You will be responsible for developing and reviewing care and support plans, coordinating service delivery, maintaining compliance, and building strong relationships with clients, families, service providers, and staff. Your role is essential in ensuring clients feel supported, safe, empowered, and respected throughout their time with Rose Aged Care.
**Responsibilities**:
- Conduct comprehensive client assessments and develop tailored, person-centred care and support plans.
- Guide clients and families in using **Home Care Package (HCP)** and/or **NDIS funding** effectively to support wellbeing, independence, and quality of life.
- Provide **support coordination services**, assisting clients to access appropriate services, navigate the care system, and exercise choice and control over their supports.
- Build and maintain strong, positive relationships with clients, families, staff, external service providers, and community networks.
- Ensure consistent, high-quality service delivery through collaboration with clinical teams, allied health professionals, and care staff.
- Support clients in accessing additional community services, supports, and advocacy as needed.
- Monitor and regularly review care and support plans, making adjustments as client needs and goals evolve.
- Maintain accurate, timely documentation and ensure compliance with **Aged Care Quality Standards**, **NDIS Practice Standards**, and organisational policies.
- Supervise, mentor, and support care workers to promote safe, ethical, and effective care practices.
- Actively participate in **advertising, networking, and community outreach activities** to promote Rose Aged Care Service and assist in attracting new clients.
- Contribute to a respectful, inclusive, and positive team-focused work environment.
**Essential Criteria**
- Relevant qualifications in **Aged Care, Community Services, Health, or Social Work** (Diploma level or above preferred).
- Minimum **1 year of experience** in a similar role involving care planning, case management, or support coordination.
- Strong knowledge of **Home Care Package (HCP) funding**, **NDIS (preferred)**, and community-based care models.
- Experience in or willingness to engage in **client outreach, marketing, and referral-building activities**.
- Empathetic, solutions-focused, and able to build rapport quickly with clients from diverse backgrounds.
- Excellent communication, time management, interpersonal, and documentation skills.
- Demonstrated ability to lead, supervise, and support a care team.
- Proficient in using digital systems, client management software, and care management tools.
- Current **driver's licence** and ability to travel to client homes across the Sydney metropolitan area.
- **Current Police Check**, **First Aid & CPR certification**, and **NDIS Worker Screening Check** (or willingness to obtain).
**Job Type**: Part-time
Pay: From $33.00 per hour
Expected hours: 21 per week
