**About the Role**

As a Care Coordinator - Aged Care, you’ll be responsible for designing and managing individual care plans that align with each client’s personal goals, health needs, and lifestyle preferences.

**Key Responsibilities Include**:

- Conducting face-to-face assessments and updating personalised care plans
- Aligning services with the Aged Care Quality Standards and client rights
- Ensuring participants receive appropriate funding and support services
- Leading and supporting a team of Support Workers
- Promoting independence, safety, and well-being

**What You Bring to the Role**
- Solid understanding of Aged Care Quality Standards and the upcoming Aged Care Reforms (July 2025)
- Demonstrated experience managing aged care caseloads
- Strong communication and people skills
- Exceptional organisational and time-management abilities
- Confidence in assessment processes and care plan development
- Familiarity with the Support at Home program

**What You’ll Need**
- Degree in Nursing, Social Work, or related field (or equivalent experience)
- Minimum 3 years’ experience in aged care or community care coordination
- Current Driver’s Licence and insured vehicle
- Valid First Aid & CPR Certificate
- Cleared National Criminal History Check and WWCC (or willing to obtain)
