**About Us**:
Triniti Home Care is a leading provider of NDIS services, dedicated to empowering people with disabilities to live fulfilling and independent lives. We are currently seeking two **full-time Support Coordinators** to join our passionate team—one in the **Newcastle-Gosford** area and one in the **Campbelltown-Macarthur** region. These roles offer a **hybrid working model**, combining work from home with community-based visits.
**About the Role**:
As a Support Coordinator at Triniti Home Care, you will assist NDIS participants in navigating the system, accessing appropriate supports, and achieving their goals. You will play a key role in ensuring participants feel empowered, respected, and well-supported throughout their journey.
No previous experience is required—we provide comprehensive training and ongoing mentoring. If you're organised, compassionate, and committed to making a positive impact, we encourage you to apply.
**Key Responsibilities**:
- Support participants to understand and implement their NDIS plans.
- Connect participants with services and supports that meet their individual needs.
- Monitor progress, update plans as required, and ensure services remain aligned with goals.
- Build strong, respectful relationships with participants, families, and service providers.
- Keep accurate, compliant, and up-to-date records.
- Advocate for participants to ensure their choices and voices are central to support decisions.
**What We’re Looking For**:
- A genuine passion for supporting individuals with disabilities.
- Strong communication, organisation, and time-management skills.
- Ability to work independently and within a team.
- A current driver’s licence and access to a reliable vehicle.
- Relevant qualifications in Social Work, Disability, or a related field are **desirable but not essential**.
- Previous experience in support coordination or the disability sector is **valued but not required**.
