**Your Role**:
Make a real impact and grow your career in a supportive, collaborative environment. As a **Care Coordinator**, you will be at the forefront of empowering individuals with complex needs to transform their lives. In the **AOD Continuing and Coordinated Care Program (CCCP)**, you’ll deliver high-quality, person-centered care, fostering positive change for clients impacted by substance use and other challenges.

**Your Responsibility**:

- Lead comprehensive assessments and develop tailored care plans with clients.
- Collaborate with referral partners and agencies to provide holistic, wrap-around support.
- Build referral pathways to connect clients with essential services.
- Provide culturally appropriate care for Indigenous Australians through relevant service links.
- Maintain detailed case notes, client records, and accurate data.
- Offer hands-on support across housing, employment, education, and life skills.

**You Will Need**:

- Relevant tertiary qualifications and significant experience in AOD and mental health care coordination.
- Proficiency in conducting assessments using standardised tools.
- Experience providing therapeutic interventions with empathy and professionalism.
- Strong knowledge of community resources and the ability to work independently and in a team.
- Commitment to client-centered care aligned with the CCCP and the Society’s AOD Model of Care.
- Strong computer literacy, especially in data collection and record-keeping.
- A valid NSW driver’s license.
