We are expanding our Support Coordination team. You will be working from home, as well as travelling to see clients in their homes when needed. Majority of our client are in the Macarthur region, but some do live further afield.

**You will need**:

- Minimum Certificate IV in Mental Health or Disability, currently studying or willingness to achieve same, or higher relevant qualification.
- Minimum 12 months Experience in previous support coordination or case management role essential, must have minimum 3 years industry or lived experience.
- Full current NSW Driver licence.
- NDIS Worker Screening check.
- Working with Children check.
- Current national police check.
- All certificates from NDIS website.
- Home office with computer and reliable internet and phone service, and a smart phone.
- Strong computer skills, with experience in Microsoft Office, video conferencing and client data management systems.
- Demonstrated ability to adapt communication to build and maintain diverse connections and relationships with NDIS participants, service providers, community contacts and staff teams.
- Excellent written and verbal communication skills, including the ability to write detailed reports.
- Knowledge of and ability to research relevant service providers and services in the areas.
- Ability to work autonomously at times prioritising duties, as well as work collaboratively with other Support Coordinators and a range of stakeholders.
- A very good understanding of how NDIS works and remain up to date with changes. An understanding of the Support coordination role including the roles parameters and limitations.
- Appropriate attitudes towards disability and mental health to be always upheld.
- Open, honest and respectful communication with a mature outlook and ability to give and receive feedback for constructive personal development.
- An understanding of billable hours and KPI’s.
- To be able to demonstrate sound understanding of professional boundaries.

**Duties and Responsibilities.**

Your day-to-day tasks include, but are not limited the following duties:

- Support implementation of all services in a participant’s plan, including informal, mainstream and community, as well as funded supports.
- The coordination and collaboration between multiple stakeholders from formal, mainstream, community, and informal supports. Chairing meetings for all stakeholders as needed.
- Networking and liaising with stakeholders across a number of platforms and conducting yourself professionally at all times as a representative of the company, encouraging repeat business.
- Research, coordinate and manage complex intertwined supports for the participant across multiple providers, ensuring client choice and control is always upheld when making referrals.
- Identify risks an implement supports in areas such a sustaining informal support, housing sustainability, risks of abuse. Following through with any necessary reporting falling under your responsibility as a mandatory reporter.
- Identify and seek relevant assessments and associated service design.
- Building clients capacity to self-direct their own supports and participate in the community.
- Identify and resolve barriers to service provision, along with managing times of crisis.
- Ensure mainstream services meet their obligations (i.e., housing, education, justice, health)
- Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency.
- Maintain professional case notes, documentation and report writing. Including following company filing structure. Understanding that your case notes can be subpoenaed in a court case or requested by the NDIS for any investigative purposes.
- Support the participant if required to access additional support through plan reviews.
