We are seeking someone who is an_experienced_ **Home Care Package Coordinator or CHSP Coordinator.** You will be responsible for the coordination, care planning and monitoring of Home Care Package clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families and other health professionals. You will work closely with their other Coordinators, Registered Nurses and Allied Health teams. This is a **PART TIME **position (20 - 25 hours per week, Monday to Friday), based in the Brisbane CBD close to public transport with company cars available for in-home visits. As a values driven organisation, we employ people who are caring, compassionate and respectful and are looking for someone who genuinely enjoys making a difference to Seniors lives and building trustful, professional relationships with them to help them remain living independently in their own home. **To be successful in this role you will have**: - A minimum of 1 years experience as a HCP Coordinator or CHSP Coordinator - Case Management Skills set or Diploma of Community Services or equivalent qualification - Current First Aid and CPR - Solid organisational skills including the ability to problem solve, prioritise and multi-task - A team player with excellent communication and interpersonal skills - Good teamwork - Good computer skills including with care databases, word, excel, outlook - Be Self-motivated with a "can do" attitude If you speak another language that will be very welcomed. You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza. **Job Type**: Part-time Part-time hours: 20 - 25 per week **Salary**: $37.00 per hour **Benefits**: - Salary packaging Schedule: - Monday to Friday **Education**: - Certificate I - IV (preferred) **Experience**: - CHSP or HCP Coordinator: 1 year (required) Work Authorisation: - Australia (required)
