The HCP Service Coordinator Team Leader, provides leadership, support and guidance to the team in scheduling and coordinating client services.

Aligned with the Consumer Directed Care framework, the key aim of a Home Care Package is to assist people in living at home for as long as possible, enabling them to have the choice and flexibility in their at home support and assist consumers and providers to maximise this choice and flexibility.

**The Service Coordinator Team Leader role will**:

- Maintaining a structure of a high performing team that achieves performance targets as efficiently as possible with a view to reducing wastage and duplication
- Monitoring productivity and performance measures using KPI’s
- Completing month end reports
- Providing feedback to team members on their performance, works collaboratively with the HCP Clinical Team Managers and People & Culture Business Partner to address team performance gaps
- Reviewing ongoing processes to enhance service delivery from the team
- Promoting and strengthen the Bolton Clarke profile as a service provider of choice through relationship management of both internal and external stakeholders by demonstrating excellent customer service principles.
- Achieving tasks allocated with the operational plan and report outcomes.

**What do we need from you?**
- Knowledge and experience of community-based care and support services
- A working knowledge of HCP funding and how it impacts on scheduling clients’ visits
- An innovative and dynamic approach to the provision of care
- Excellent communication and interpersonal skills
- Proven ability to contribute to and, work as part of a dynamic team in addition to operating independently
- Demonstrate alignment with organisational values and ethics
- Excellent organisational and time management skills and ability to work within limited timeframes
