BHN (Better Health Network) is a leader in the Community Health sector, supporting the inner south and bay side suburbs of Melbourne.

As one of the first points of contact, you will provide effective screening and referral services to our clients, maintain confidentiality of sensitive client information, and build a positive rapport with a wide range of people, including clients, their family, friends and / or carers. The main purpose of this role is to support our clients to make the most appropriate link to our services and programs across BHN with professionalism, responsiveness, and a person-centered approach. You will do this by providing quality customer service on-site and over the phone to our clients, workforce, and community members.

Multiple roles available, mix of permanent full time or part time. Flexible working conditions including working from home or multiple sites across BHN. Immediate start available.
Extensive training will be provided onsite and is ongoing for at least the first six months.

**Key Responsibilities**
Intake Duties
- Manage appointments for clients and support them with referrals to other services where appropriate.
- Maintain strict confidentiality in accordance with organizational policies and procedures.
- Collect all information sensitively, considering cultural, language, special needs, privacy, and confidentiality requirements.
- Record all mandatory client details accurately in our client management system.
- Assist with smooth client entry processes in collaboration with different services.
- Liaise with other service agencies and participate in Service Coordination working groups.

**Ideally, you will possess**:

- Experience in a multidisciplinary healthcare environment.
- Excellent communication and IT skills.
- Strong attention to detail and multitasking abilities.
- Problem-solving and decision-making skills.
- Demonstrated intermediate experience in the proficient use of Microsoft Office Suite.
- Excellent written and oral communication skills with a wonderful phone manner.
- Demonstrated high level of discretion and sensitivity.
- Well-developed time management and organisational skills.
- Empathy and caring nature.
