**About the Role**:
As an Intake Officer, you'll manage and assess client referrals, update service information via the My Aged Care Provider Portal, and complete annual reviews. Your role is pivotal in enhancing the quality of life for our clients by ensuring they receive the care and support tailored to their needs.

**Key Responsibilities**:

- Manage client referrals and service information updates.
- Conduct needs-based, client-centered assessments.
- Facilitate referrals to HCP program with client consent.
- Maintain client confidentiality and privacy in accordance with legislative requirements.

**Who We're Looking For**:

- Passionate individuals with a genuine interest in enhancing the lives of others.
- Computer literacy, including Microsoft Office and client database management systems.
- Desirable: Minimum of 3 years’ experience in an assessment role, preferably in Community Services/Healthcare. Relevant qualifications are a plus.
- Essential: Current required vaccinations, a valid driver’s license, NDIS Worker Screening, WWCC, and the right to work in Australia.
