Our disability services include community Participation, Capacity Building, Accommodation programs, Mental Health Care, Nursing Care, - Behavioral Care, Support Coordination, Community Support Service, and in-home support services.

**About the exciting role**

PDC Services is looking for a full time **qualified and experienced NDIS Support Coordinator** to join our team with a strong understanding of the NDIS and a passion to help people.

The successful applicants will be responsible for establishing positive collaborative relationships with NDIS participants. You will work with individuals to navigate through the complexities of the NDIS and engage different supports utilising the participant's NDIS funding.

You will provide information and assist the person to identify and coordinate support to connect with local communities, build skills, overcome barriers, help achieve goals and assist with NDIS plan reviews and reporting.

**The position requires an individual who has**:

- Experience providing NDIS Support Coordination services.
- Nursing qualification and experience, highly desirable.
- Excellent time management and administration with the ability to maintain accurate, professionally appropriate and current records and written reports.
- Effectively utilise NDIS packages by using your knowledge of the NDIS, the service system, being assertive, and getting results
- A solid understanding of the NDIS and its importance to people with disability and their support networks.
- Demonstrated experience with facilitating multi-disciplinary coordinated planning and develop collaborative relationships and networks between service providers.
- Excellent note taking and record keeping of participant progress.
- Understanding of NDIS reporting process.

**Job Type**: Casual

**Salary**: $59,812.19 - $70,000.00 per year

Schedule:

- 8 hour shift

- Brunswick VIC: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
