Responsible for determining eligibility and assessing the individual needs of veterans for home care services in a timely manner. This includes conducting assessments using the phone-based Veteran’s Home Care (VHC) standard assessment tool, completing accurate and timely documentation, coordinating follow-up referrals and service reviews, maintaining up-to-date knowledge of community-based health and support services, and contributing to quality improvement activities.
This role delivers a phone based service for eligible Veterans located in Western Australia from our office located in Ringwood, Victoria. Hours of work for this role will be Western Australian business hours.
**To be successful, you will require;**
- Minimum two years’ experience in the aged or community care sector
- Comprehensive assessment and care planning skills
