- $ per hour (SACs), $55.98 per hour (Nurses) or $ $ per hour (Dependent on qualifications and experience) (Allied Health)

Responsible in determining eligibility and assess the needs of individual veterans for home care services in a timely way, using the phone based Veteran's Home Care (VHC) standard assessment instrument; complete relevant documentation; arranging follow-up referrals and review of services; maintaining knowledge of community based health and support providers and participation in quality activities.
This role delivers a phone-based service for eligible Veterans located in Western Australia and Tasmania from our office located in Ringwood, Victoria. Hours of work for this role will be Victorian (AEST) business hours.
To be successful, you will require;
- Minimum two years' experience in the aged or community care sector
- Comprehensive assessment and care planning skills
- A current Working with Children Check
Desirable
- Tertiary qualification in health-related discipline.
- Discipline specific registration where required.
