Care Connect is one of Australia's largest, trusted, not-for profit home care specialists, operating across New South Wales, Victoria, and Queensland. Since 1994, we have helped our clients to live independently at home, with a planned and coordinated approach to care needs through independent advice and choice of services.

Helping clients live happily and safely at home for life is at the heart of what we do. We work together with our clients to enable them to identify what their needs and goals are, and then connect them with the care and support required through our extensive network of external service providers.

Care Connect are moving in an exciting new direction, with a planned move to build out a national, directly managed team of care workers to deliver services to our clients. This will supplement the existing arrangement of contracting external specialist partners to deliver our clients' care services.

Project Diamond has been established to implement this new business model in a phased approach.

**About the Role**

The Client Referrals Coordinator is a newly created role on Project Diamond to drive the active, ongoing process of identifying clients to offer our services to, and work with the business to oversee the initial scheduling of services (new clients) or the transition of service from a brokered partner provider to our own team (existing clients).

We are seeking a highly motivated individual with strong excel knowledge who will be able to adapt to the methods of analysis and project delivery as required for the project. You will work collaboratively across the organisation whilst also:

- Engage with the Home Care Advisers, Service Coordinators and Care Managers to promote Care Connect's new direct care service
- Develop tools and processes to optimise the referrals process and ensure an efficient flow of referrals
- Sponsor the development of internal and external communication to support client awareness and adoption of the service
- Develop tools and processes to optimise the referrals process and ensure an efficient flow between Service Coordination and the Direct Care team, adjusting as required with a test and learn mindset
- Perform reporting against KPIs as required

- Outstanding client coordination and customer service skills
- Excellent interpersonal skills and the ability to link people into a range of supports & services
- Well developed, fast and accurate data entry skills
- Ability to clearly communicate ideas on complex and sensitive issues in writing or verbally, selecting language, tone and format to suit the audience
- Ability to learn quickly and adapt to changing business environments
- Knowledge of community and health care resources and services networks OR experience in the community/health sector with care coordination - preferred
