With growing business needs and clients in different states, Alpha Omega Management Consulting is urgently looking for a full time Aged Care Coordinator in Melbourne, Victoria.

**Position purpose**:
Reporting to HCP Team Leader, the role is to ensure the smooth running of the business through the provision of end-to-end client services management to assist them in achieving their goals set out in their individual plans.

Job responsibilities:

- End to end client services management: intake, service (and risk) assessment, reviews, service transfer and coordination of direct support activities (working with Rostering Officer) and understanding customer’s monthly statement.
- Participate in company after hours programs.
- Working with customers, their supports and other providers (e.g. LAC, ACAT) to achieve their goals in line with their individual plans.
- Maintain accurate and up to date case notes on customer progress.
- Maintain an up-to-date knowledge of local services and pathways that customers can access and current sector systems and practices.
- Facilitate care worker support and supervision including accurate documentation, record keeping.
- Maintain internal filing, including accurate client documentation and other miscellaneous filing.
- Complying with all relevant legislation.

Essential Criteria:

- Relevant tertiary or vocational qualifications in community and social service management or equivalent.
- At least one-year experience in the similar role / industry.
- Demonstrated skills and experience working with case management workforce (clinical and non-clinical) within a community setting, managing complex client care scenarios.
- Excellent interpersonal, influencing, negotiation and facilitation skills and proven ability to successfully engage a diverse range of stakeholders.
- Commitment to managing service delivery within a client focused model of care.
- Well-developed IT skills and experience with digital solutions.
- Driver’s license and ability to travel.
- Ability to speak fluently a second language such as Arabic, Chinese is desirable.
