**About the role**
In this role you will be responsible for the efficient and effective delivery of assessment and review services for people eligible for Commonwealth Home Support Package (CHSP) services and the coordination of service delivery for clients receiving CHSP services, ensuring that the assessed needs of eligible aged people, people with dementia, people with disabilities and their carers within our community, are met

This position is also responsible for the assessment of younger people eligible under the Home and Community Care Program for Young People (HACC PYP).

Key responsibilities include:

- Manage and action referrals and undertake assessments using the mandated National Screening & Assessment Form (NSAF).
- Create goal and client directed Support Plans by adopting a wellness and reablement approach to support clients for the purpose of maximising their independence and autonomy.
- Match and refer clients to provider(s) using the online Gateway System Service Finder for services under the CHSP and to other services not included within this functionality.
- Provide Linking Service Support for clients with high levels of vulnerability and multiple care needs to access a range of aged care and other services.
- Develop and maintain effective referral, assessment and review mechanisms for prospective and existing service recipients.
- Ongoing monitoring of clients, appropriateness of service arrangements, and undertake regular reviews.
- Ensure clients and their carers are informed of rights and responsibilities, type and levels of care to be provided, the costs involved and grievance procedures.

**About you**
To be successful in this role, you will be experienced in assessment practice and the preparation of Care Plans, as well as knowledge of issues relating to older people and people with disabilities. You will have the ability to liaise with a diverse range of people, have a professional and empathetic approach to your work and enjoy dealing with complex/challenging situations.

Essential Experience:

- Awareness and support of the philosophies of healthy ageing, promotion of independence, non-intrusive care and general disabilities.
- Extensive knowledge of governmental and community resources available for older people, people with dementia, people with disabilities and their carers.
- Comprehensive knowledge of holistic assessment and care management practices.
- Knowledge of funding, HACC PYP and Regional Assessment Services (RAS) guidelines
- Knowledge of comprehensive assessment and review practices and the preparation of Care Plans and service delivery plans.
- A minimum of Diploma in Aged Care, Community Services or other tertiary qualification in Human Service field or relevant experiences
