Nunkuwarrin Yunti, the foremost Aboriginal Community Controlled Health Organisation in Adelaide aims to promote and deliver improvement in the health and wellbeing of all Aboriginal and Torres Strait Islander people and their families in the greater metropolitan areas of Adelaide and regional areas to advance their social, cultural and economic status. Nunkuwarrin Yunti is a Child Safe Organisation.

The program will address the needs of children, young people, and families to provide an early intervention program that seeks to interrupt intergenerational disadvantage through thievery of targeted activities that promote safety and wellbeing.
- The primary focus of Strong families Strong Communities Care Coordinator is to:

- Engage with referred families to build a therapeutic relationship based on the strengths and needs of the family.
- Walk alongside the family to navigate the service system to find and access services that enabled the family to address the challenges that they are facing, ensuring access and feedback loops regarding referral outcomes.
- Development of Case Plans in partnership with the family identifying needs and risks, which lead to family led goal setting.
- Provision of practical and therapeutic support in the home with flexible hours of delivery.
- Support adult caregivers to connect with volunteer, education, training and employment opportunities, including peer support, leadership, and mentoring activities.
- Undertake, broker and/or facilitate access to support networks, aftercare and self-help groups and community activities, and assist clients to build/access these networks.
- Liaise, advocate and broker services between clients and internal and external stakeholders as necessary.
- Establish and maintain collaborative and productive working relationships with current and potential stakeholders and partners, both internally and externally.
- Liaise and consult with other services to develop and support the provision of recovery and harm reduction orientated treatment that aligns with the client’s individual treatment plan.

**Desired Skills and Experience**:
**ESSENTIAL - includes qualifications, skills, experience, and knowledge.**
- Certificate 4 or equivalent in one of the following areas: Aboriginal primary health care, community/social health services with a minimum of two years vocational.
- Experience in providing effective and well-informed advice, assessment, care coordination, advocacy, liaison, and referral to community services.
- Well-developed written and verbal communication skills and the ability to communicate effectively with Aboriginal people.
- Ability to liaise, network and negotiate with a range of human service organisations and government agencies.
- Demonstrated experience such as health promotion, counselling, case management, program coordination and delivery.
- Ability to deal with people with high and complex needs and the ability to deal with conflict constructively.
- An understanding and knowledge of social, health and cultural issues effecting the Aboriginal and Torres Islander population in Southern Adelaide.
- Ability to work with Aboriginal and Torres Strait Islander communities and their leaders, respecting traditional culture, values, and ways of doing business.

**DESIRABLE**
- Previous experience working in community primary heath projects/programs that demonstrate best practice for Aboriginal Torres strait Islander people.
- A comprehensive understanding of contemporary Aboriginal and Torres Strait Islander family dynamics and kinship systems
- Strong commitment to harm minimization principles and understand addictive behaviours, in particular people who inject drugs.
