**This opportunity**:
Our residential site, located in** Leabrook** is currently seeking a senior Registered or Clinical Nurse, to take up the role of **Care Coordinator**. This is an opportunity to join a caring and supportive environment, and make a difference to the quality of care we deliver to our Resthaven residents.

**A little more about the role**:

- Working collaboratively with the Manager Residential Services and the management team to provide quality care and services to all residents and their families
- Working in consultation with residents and families in the planning, delivery and evaluation of clinical care
- Delivering professional standards of contemporary best practice nursing and care services at your site
- Ensuring effective and collaborative working arrangements with visiting medical practitioners and other allied health professionals
- The supervision of nursing staff and personal care assistants

**About you**:

- You will have outstanding leadership skills and have the ability to motivate staff to achieve organisational goals
- A demonstrated ability to communicate effectively with staff and clients
- Ability to logically prioritise varying work demands
- Strong organisational and time management skills
- You have an understanding of the ageing process and a commitment to caring for aged clients
- An understanding of aged care funding processes and required documentation including the ACFI
- Registration with AHPRA is essential
