**JOB DESCRIPTION**:Client Services/Program Coordinator

**TYPE OF EMPLOYMENT**:Part Time

**REPORT TO**:Director

**LOCATION**:Payneham

**Role**:
The Services/Program will be instrumental in facilitating the transition to the National

Disability Insurance Scheme (NDIS) and provides hands-on leadership to support people achieving their goals.

**Duties and Responsibilities**

**Client Services**
- Provides coaching and direction to establish and maintain, within the team, supports and services to assist people to live with dignity at AEON’s accommodation with complete choice and control.
- Contributes to the delivery of quality of services.
- Promotes a clear vision of quality in services and supports, reviewing and monitoring quality and leading innovation and continuous improvement
- Works closely with clients and support workers, prioritizing and matching referrals to the most appropriate support workers to ensure the development of self-determination, effective individual plans, service design, continuous review and improvement of quality services is achieved.
- Provides and check reports internally and externally as required
- Develops good working relationships with referral agencies, other providers, and community groups
- Develops and manages internal and external relationships with external providers team, linking customers and the team for day-to-day activities. Uses skills across all the accommodations and provides training and mentoring to others outside the area when required.
- Works with the managing director to develop strategy and plans to deliver excellent support to the current base of individuals and ensure the continued growth and expansion of home services.
- Ensures that people’s options for support are explored, referrals are passed to, and acted on, by the most relevant support worker within the area.
- Contributes to strategic and business planning and or leads projects and produces reports as required,
- Maintain own professional development.

**Program Coordinator**
- Support planning and facilitation of day options program for people with disabilities and its activities
- Ensure implementation of Company policies and practices
- Manage effective communications with family/guardians
- Help build positive relations within the team and external parties
- Schedule and organize meetings/events and maintain agenda
- Create a monthly activity calendar to support a diverse range of clientele
- Oversee daily activities planner and organise bookings
- Ensure technology is used correctly for all operations (video conferencing, presentations etc.)
- Prepare and document all client intake forms, records, progress notes and other paperwork
- Ordering and procuring materials for the programs
- Support clients to achieve their goals, skills development and implement support plans

**Skills, Attributes and Experience**

**Essential**:

- Comprehensive understanding of community networks and relevant legislation, such as the NDIS.
- Demonstrate experiences and understanding of individualized assessment approaches.
- Highly developed interpersonal, communication and negotiation skills, with the ability to develop relationships with a wide range of stakeholders.
- Varied and relevant experience in community services with an emphasis on the design of innovative, flexible and holistic individualized plans and associated costings.
- Broad knowledge of finances processes and guidelines related to individual funding and budgets and high level conceptual and analytical skills to assess and develop same
- Sound organizational skills with the ability to work independently, meet deadlines and manage competing priorities
- Knowledge of key reform agendas currently impacting the community services sector.
- Knowledge of program management and development procedures
- Detail-oriented and efficient

**Desirable**
- Diploma or Certificate IV in Community Services or equivalent

**Conditions of Employment**

**Job Type**: Part-time
Part-time hours: 16 per week

**Salary**: $65,000.00 - $85,000.00 per year

Schedule:

- 8 hour shift
