Purpose of the Role:

The Service Coordinator will oversee the Team Leaders and Disability Support Workers, ensuring all services meet the NDIS Practice Standards and the needs of participants. This role will foster innovation and collaboration among the team to deliver creative, participant-focused services while supporting the organisation's growth.

Key Responsibilities:

  1. Team Leadership and Oversight:

  2. Provide mentorship, guidance, and support to team leaders and disability support workers to ensure optimal service delivery.

  3. Monitor and evaluate service compliance with NDIS Practice Standards and implement continuous improvement measures.
  4. Take a proactive approach to recruitment, rostering, and staffing sustainability, including a workforce training plan.
  5. Conduct regular team, one-on-one, probationary, and annual performance review meetings with disability support workers and team leaders.
  6. Participant-Centric Planning:

  7. Develop and routinely review personalised support and risk assessment plans, incorporating participant needs, goals, and input from health and allied health teams.

  8. Collaborate with the Lifestyle Team to design and implement creative, engaging, and inclusive activities tailored to participants' needs and goals.
  9. Advocate for participant feedback to refine services and promote satisfaction.
  10. Participate in the on-call roster to provide after-hours support as needed.
  11. Collaboration and Networking:

  12. Liaise with the Business Development Manager to identify opportunities for participant growth and organisational partnerships.

  13. Collaborate with the Business Development Manager to develop new services, identify opportunities to improve customer onboarding procedures, and increase conversion rates.
  14. Represent Inclusive Lifestyle at networking events to promote our services and establish strategic alliances.
  15. Compliance and Reporting:

  16. Ensure adherence to industry updates and changes in practice standards, communicating these effectively to relevant staff.

  17. Overseeing and collaborating with the Operations Manager to ensure the effective management of the incident reporting system and its full compliance with NDIS Incident Management guidelines and standards.
  18. Liaise with team leaders to monitor, track, and report the use of restrictive practices in services in accordance with the NDIS Restrictive Practice Guide.
  19. Prepare detailed reports on service delivery, compliance metrics, and team performance for the Operations Manager.
  20. Ensure all participant documentation, including care plans, progress notes, and financial records, is complete and accurate.
  21. Team Collaboration and Innovation:

  22. Foster a culture of collaboration and creativity, encouraging shared ideas and strategies to enhance service quality.

  23. Collaborate with the management team to analyse trends, identify service deficiencies, and propose efficiency and development initiatives for organisational advantages.
  24. Lead brainstorming sessions and training workshops for staff development.

Selection Criteria:

  • Essential:

  • Relevant qualifications in disability/community services, social work, psychology, developmental education, nursing/enrolled nursing or a related field.

  • Comprehensive knowledge of the NDIS and the ability to navigate its systems and standards.
  • Proven experience in team leadership, service coordination, or a similar role.
  • Strong interpersonal and communication skills, with the ability to engage effectively with diverse stakeholders.
  • Demonstrated problem-solving ability and a proactive approach to challenges.
  • Desirable:

  • I have extensive experience in NDIS service delivery, NDIS Incident report management, managing teams and networking.

  • I possess a creative mindset and the ability to design engaging activities.
