This is an exciting leadership opportunity for a highly motivated and experienced Allied Health professional to join the Western Health at Home team as a Service Manager for the Specialised Rehabilitation program. This role is a fixed term, full time position for 4 months.

The Specialised Rehabilitation Program (SRP) is an outpatient service which aims to provide timely access to assessment and rehabilitation following a recent illness or injury. The service provides short-term, multidisciplinary, and goal-oriented therapy that optimises recovery and allows clients to spend more healthy days at home.

**The objective of the Specialised Rehabilitation Program is to enhance Best Care by**:

- Maximising recovery by delivering patient centred, goal-oriented therapy.
- Optimising independence and more healthy days spent at home
- Providing patient education with an emphasis on self-managment
- Reducing avoidable readmissions by improving function and implementing sustainable discharge plans
- Are experienced or emerging leaders looking for a new challenge
- Possess strong communication and collaborative teamwork skills
- Demonstrate highly developed problem solving and lateral thinking skills
- Will act as a role model and mentor for the clinical team

**To be Successful in this Role you must**:

- Have appropriately recognised tertiary qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Demonstrate highly developed leadership skills and experience leading teams
- Demonstrate highly developed communication and interpersonal skills
- Ensure safe and high-quality patient care is delivered in line with Western Health values and Live Best Care framework
- Contribute to a culture that values ongoing learning and be part of a positive workplace
- Have a valid Victorian Drivers Licence
