**Position Summary**
- Improve the patient experience by providing care in the comfort of their own home
- Assist patients to achieve their care goals and best possible level of health and function
- Promote and support safe and early discharge from hospital settings
- Are experienced or emerging leaders looking for a new challenge
- Possess strong communication and collaborative teamwork skills
- Demonstrate highly developed problem solving and lateral thinking skills
- Will act as a role model and mentor for the clinical team

**To be successful in this role you must have**:

- Appropriately recognised tertiary qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Highly developed interpersonal, written and electronic communication skills
- Computer literacy commensurate with role
- Capacity to be innovative, resourceful and adaptable to change
- A current Victorian Driver’s Licence
