An exciting opportunity currently exists for a Social Worker to work in a permanent part time (0.6EFT) basis within the Chronic and Complex Care division at Western Health.
The Rapid Allied Health (RAH) team aim to prevent avoidable hospital admissions and presentations to ED by providing a rapid response and short term Allied Health Intervention. RAH accepts referrals from (but not limited to): Emergency Department Care Coordinators, Internal WH wards, External Hospitals, General Practitioners, Medical specialists, and Ambulance Victoria. The RAH team consists of occupational therapists, physiotherapists and social workers and is integrated with the ACE ED Care Coordination team.
The Community Care Coordination team aims to:
- provide an alternative to hospital presentation and admissions
- effectively coordinate the care of people with Chronic and complex conditions
- promote and support safe and early discharge from inpatient settings (collaboration with inpatient Care coordinators)
- provide support for patients to remain at home following a change in health or functional ability

**To be Successful in this Role you
- have at least 3 years social work clinical experience
- will have a passion for providing high quality patient centred care to people with acute and chronic health conditions living in the community
- have excellent written and verbal communication skills
- will enjoy working in and contributing to an interdisciplinary team
- need a current Victorian drivers licence
