We are seeking a highly motivated and compassionate Intake & Client Services Officer to join our team in Werribee. This pivotal role will be instrumental in ensuring our participants receive personalised and timely services that promote their well-being.

**Key Responsibilities**:

- **Intake Excellence**: Conduct comprehensive intake assessments, identifying participant needs and goals to develop tailored service plans within established timeframes.
- **Seamless Onboarding**: Facilitate a smooth transition for new clients by managing documentation, service agreements, and coordinating with external agencies.
- **Empowering Support**: Oversee the development and implementation of responsive support plans that align with individual needs and NDIS guidelines.
- **Risk Management**: Conduct thorough risk assessments and implement safety protocols to ensure the well-being of both staff and clients.
- **Thorough Documentation**: Maintain accurate records, prepare detailed reports, and advise on service options, referring clients and families to appropriate agencies as needed.
- **Collaboration**: Actively participate in team meetings, contributing to a positive and supportive organisational culture.

**Selection Criteria**:

- **NDIS Expertise**: Demonstrated understanding of the NDIS framework and the diverse needs of individuals with disabilities.
- **Autonomy & Teamwork**: Proven ability to work independently and collaboratively within a fast-paced environment.
- **Intake Leadership**: Experience developing and leading intake processes, with knowledge of state and federal support systems.
- **Strong Communication**: Excellent interpersonal, written, and verbal communication skills to effectively interact with clients, families, and stakeholders.
- **Problem-Solving & Analysis**: Ability to think critically, analyse complex situations, and find creative solutions.
- **Professionalism & Discretion**: High level of professionalism, integrity, and confidentiality.
- **Tech Savvy**: Proficiency in database management and other relevant software.
- **Client-Centric Focus**: Flexibility, empathy, and a genuine commitment to client-centered service delivery.

**Qualifications**:

- Tertiary qualifications in Social Work, Disability Studies, Community Services, or a related field are highly desirable.

**Additional Requirements**:

- Willingness to travel frequently within the region to meet with potential and existing participants.
- Current and valid Police Check, Working with Children Check (WWCC), Driver's License, and NDIS Worker Screening Check are mandatory.
