**About the roles**

GEO Healthcare’s Post-Release Support Coordinators (PRSC’s) will work as part of a multidisciplinary Clinical Services team, providing post-release support to people leaving prison with complex health needs.

PRSC’s will have a background in social work and knowledge of the health service system. They will understand the difficulties experienced by people leaving custody and the need for follow-up support.

Following their release from prison, people with complex health needs will be contacted by a PRSC to encourage and support ongoing engagement with their health, mental health and/or AOD service providers.

An opportunity exists for a full-time Post-Release Support Coordinator to work at the Bridge Centre, Richmond Victoria.

- Position Description - Post Release Support Coordinator

**Skills and Experience**:
**Essential**:

- Degree qualification in relevant discipline, eg. Nursing, Psychology, Social Work, Occupational Therapy
- Registration with the relevant professional board or association (Australian Health Practitioners Regulation Agency, (AHPRA), Australian Association of Social Work (AASW))
- Knowledge of the health service system, acquired in a professional setting.
- Knowledge of the post-release support needs of people released from prison, acquired in a professional setting.
- Demonstrated experience in person-centred care and recovery-focused support for people with complex health needs.
- Strong verbal and written communication skills and an ability to build and maintain positive collegial and stakeholder relationships.
- A commitment to professional development through participation in further training, supervision, and mentoring.
- Knowledge of and previous experience in the Criminal Justice System including the role of the Courts, prisons, parole and community corrections would be well regarded
- Demonstrated ability to work collaboratively with others both cross-agency and as part of a multidisciplinary team of professionals

**Other Requirements of the role**:

- Valid Australian driver’s licence.
- Ability to work within a prison setting, which will involve direct contact with people in prison associated with the delivery of healthcare.
- Satisfactory completion of a National Criminal History Check.
- Ability to obtain and maintain a security clearance.
- Working outside of ordinary business hours may be required.
