**What You Will Be Doing**

The Community Care Allied Health Team Leader role sits within Peninsula Health's Community Health Independence Programs. The Team Leader is responsible for leading a multidisciplinary team towards the delivery of Care Coordination and Allied Health intervention within their scope of practice, to clients who have chronic health conditions and/ or complex healthcare needs.

The Allied Health Team Leader will ensure clinical leadership is delivered in relation to consultation, education, training and workforce development to staff in order to

enhance the capacity of the service and capability of the staff to work with people who have chronic and/or complex healthcare needs.

The Allied Health Team Leader will contribute to a supportive and inclusive work culture that embraces diversity, and commits to upholding Peninsula Health's values and broader organisational goals.

**What You Need**

We anticipate you will have:

- Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation.
- Demonstrated leadership experience, including n providing clinical supervision.
- Demonstrated ability to manage multiple competing tasks and effectively prioritise.
- Demonstrated experience working with people who are at-risk and vulnerable and have complex needs.
- Specialist experience and knowledge regarding chronic disease management.
- Demonstrated understanding of quality improvement and change management processes.
