This role sits within the Disability services team at Peninsula Health based at Frankston. We are a supportive multidisciplinary team within Peninsula Health, which is a major metropolitan health service caring for a community of 300,000 people. We love working in the beautiful Mornington Peninsula and Frankston surrounds, renowned for its Mediterranean lifestyle where many of our staff enjoy an excellent work life balance.

**What You Will Be Doing**

This position presents a great opportunity for an experienced Support Co-Ordinator with a degree in Community Development, Nursing, Occupational Therapy, Speech Pathology or Physiotherapy to join the Support Co-ordination team. We are a multi-disciplinary team and we co-ordinate supports in collaboration with our participants with disabilities. We are passionate about high quality care and being an employer of choice.
