DAACCS provides support to people with physical, intellectual, and psychosocial disabilities and elderly people living in their own homes so that they are fully included in the mainstream community. This support ranges from 1:1 individual support to group support. Support is provided at home or in the community to meet individual needs.

Services such as coordination of support and case management are delivered to individuals in line with their individual plans.

Service Information

DAACCS service delivery is typically staffed by multi-lingual / bi-lingual support workers. These services include respite care, domestic assistance, home care, skills development, social support, mental health support, and travel/transport arrangements.

Position Purpose

The Support Worker provides high-quality, person-centred care and support to individuals with disabilities and elderly clients in line with their individual care plans. The role focuses on promoting independence, ensuring safety, and enhancing well-being by assisting clients with daily living activities, personal care, and community engagement.

Key Result Area 1 - Client Care and Support

1. Provide person-centred care that promotes the independence and well-being of clients.

2. Assist clients with personal care tasks, including bathing, dressing, grooming, and toileting.

3. Support clients in performing daily tasks such as meal preparation, housekeeping, and medication management.

4. Assist clients with mobility, including the use of mobility aids, transfers, and positioning.

5. Facilitate and support clients’ participation in social, recreational, and community activities.

6. Maintain accurate records of services provided and any changes in clients’ conditions.

7. Ensure thorough understanding of direct support requirements, including participants' active support plans, positive behaviour support plans, risk assessments, medication management plans, and personal emergency evacuation plans.

Key Result Area 2 - Health and Safety

1. Follow all workplace health and safety protocols to ensure the well-being of both clients and staff.

2. Identify potential hazards in the client’s environment and report any risks to management.

3. Adhere to infection control procedures, including the proper use of Personal Protective Equipment (PPE).

4. Ensure occupational health and safety protocols are adhered to, and risks for staff, participants, and visitors are assessed and managed.

5. Provide emergency assistance as required and escalate incidents to the Service Delivery Manager promptly.

Key Result Area 3 - Communication and Documentation

1. Build and maintain positive relationships with clients, their families, and other stakeholders.

2. Report any changes in clients’ physical or emotional conditions to the Service Delivery Manager or healthcare professionals.

3. Maintain confidentiality and adhere to privacy requirements in all communications and documentation.

4. Accurately document daily activities, care provided, and any incidents in the appropriate systems.

Key Result Area 4 - Teamwork and Collaboration

1. Work collaboratively with other support staff, healthcare professionals, and management to provide comprehensive support to clients.

2. Participate in team meetings, training, and continuous improvement activities.

3. Provide feedback and suggestions to improve service delivery and enhance client outcomes.

Key Result Area 5 - Compliance and Continuous Improvement

1. Maintain a thorough working knowledge of relevant DAACCS policies and procedures and work in a manner consistent with these.

2. Ensure compliance with the National Disability Insurance Scheme (NDIS) Quality and Safeguards Commission standards.

3. Behave in ways that present a positive and professional image of DAACCS when dealing with participants.

4. Participate in ongoing training to stay current with best practices in disability and aged care support.

5. Contribute to the continuous improvement of care practices and policies by providing feedback and suggestions.

Key Result Area 6 - Administration

1. Report all incidents in accordance with DAACCS policies and procedures.

2. Ensure timely completion of mandatory training requirements and professional development identified.

3. Maintain thorough knowledge of emergency evacuation plans and participate in fire and evacuation drills.

4. Participate in the complaints and feedback process, ensuring that all complaints are reported to management and outcomes are implemented.

5. Use electronic systems to record and capture all data to complete all observations, case notes, and reports in a timely manner.

6. Contribute to a culture of continuous improvement through self-assessment and gaining feedback from participants and colleagues.
