**Graceville Centre** is an integral part of the healthcare community on the Sunshine Coast. We offer comprehensive Mental Health and Disability services. Located in Nambour on the Sunshine Coast, we strive to support our clients to improve the quality of their lives and live a life that is personally meaningful.
**About the role**:
Reporting to the Client Services Manager, the **Team Leader - Support Coordination** provides leadership and support to a small, passionate team within their allocated programs across the Mental health and Disability streams.
**In addition you will**:
- Assist in the recruitment and performance appraisals of staff
- Assist in assessment of clients
- Demonstrate appropriate skill in handling complex caseloads and mentoring staff
- Stakeholder management and effective representation at forums
- Ensure efficient and effective day to day running of the service
- Ensure collaboration across the program and ensure delivery of a successful function
**About you**:
To be successful in this role you will possess a tertiary qualification in Human Services delivery and/or significant experience in a similar or same role. In addition to this skill set you will have:
- Demonstrated understanding of complex case management.
- High level experience in delivery of services
- A current Australian driver licences
- Compliant with the Disability Services Act 2006
