Team Leader (Clinical) – Nambour | Full-Time

About the roleThis permanent full-time role is based in Nambour, supporting Participants with complex medical needs in a Supported Independent Living (SIL) home and Individual Supports (IS – In Home). You will be responsible for hands-on care, clinical oversight, and leading a team of support workers to ensure safe, respectful, and meaningful support for our Participants.
Your leadership will help shape a positive home environment that upholds dignity, independence, and quality of life. Each participant has unique and complex needs, requiring compassionate care, clinical expertise, and thoughtful leadership.
What You'll Do - Key Responsibilities
Clinical & Direct Support
Provide hands-on care and clinical oversight, including:
- Medication administration
- Catheter and continence management
- Seizure and epilepsy support
- Diabetes monitoring and support
- Respiratory care, including aspiration pneumonia prevention
- Dementia and dysphasia support
- Bowel care (including enemas)
Leadership & Team Development
- Lead, coach, and mentor a team of support workers
- Foster a safe, collaborative, and strengths-based team culture
- Train and supervise staff in clinical best practices and support protocols
Operations & Coordination
- Manage daily operations of the SIL home, including rosters, incident reporting, medication charts, and shift documentation
- Provide clinical oversight, training and supervision to IS teams
- Liaise with families, health professionals, and stakeholders
- Ensure compliance with NDIS Quality & Safeguards and relevant health regulations
Community Engagement
- Build relationships with local networks, support coordinators, and healthcare professionals
- Advocate for Participants and contribute to service growth
About you
You are an experienced and confident clinical practitioner who thrives in a leadership role. You are passionate about disability support, and you bring emotional intelligence, integrity, and resilience to your work.
You'll need:
· Current registration as a Registered Nurse (RN) or Enrolled Nurse (EN) with relevant clinical experience in community or disability care.
· Proven ability to manage complex health needs
· Experience working with or supporting people with:
o Intellectual and physical disabilities
o Non-verbal communication
o Experience with lifting and hoist transfers
o Restrictive practices
· Strong documentation and medication administration skills.
· Excellent time management and decision-making under pressure.
· Commitment to person-centred, trauma-informed, and culturally sensitive practice.
Required Certifications (have or willingness to obtain):
· Certifications MAYBO or equivalent behaviour support training
· First Aid and CPR (current)
· Manual Handling
· Medication Administration
· NDIS Worker Screening Check
· Working with Children Check
