**Description**:
About the Role:
**Job Role **- Aged and Disability Support (Community Services and Development)

**Location**: Western Sydney (25-45 km from CBD)

**_ Driver’s license and Vehicle needed._**

**Start date**:ASAP Please

**Rate per Hour**: $30 - $34.99 per hour Contract/Temp

**Shifts**: Shift/Roster (3-12hrs/week)

**Duties**:
**Complete documentation as required. This may include**:

- Customer notes
- Shift reports
- Learning Logs / Monthly reports
- Risk management / Positive behaviour

**Maintain effective communication with clients**:

- Implement and review of Individual Program Plans.
- Record the progress of each customer in line with relevant documentation.
- Provide transport for customers or assistance to use public transport.

**Provide physical assistance as required. This may include**:

- Assistance with toileting.
- Transfers/hoisting between toilet, wheelchair, motor vehicle, etc.
- Medical/special procedures where required.
- Administering medication where required.
- Physical assistance to participate in activities.
- Housekeeping duties.

The may be times where Clients will need to be taken to appointments and meetings etc.

**Many more duties advised in Interview.**

**COVID JABS** - Three Vaccinations are Required.**Tickets required**
- Certificate in Disability, Aged, Community or Individual Care
