The Salvation Army Australia’s Homelessness Service provides high quality, person-centered services to people experiencing or at risk of homelessness. We offer a holistic response to homelessness including outreach case management support, financial assistance, connection to community, and referral to internal and external specialist services.

We are seeking a values-driven Homelessness Case Manager for the Hunter region, working alongside existing Salvation Army programs. You will provide a variety of Outreach Case management support to all cohorts experiencing or at risk of homelessness and support local Salvation Army services in their response to homelessness.

The role you could play
- Provide outreach case management support to people experiencing, or at risk of, homelessness.
- Develop Case Plans alongside the participant and tailor them according to individual goals, background, and needs
- Develop Case Plans for dependents individually or incorporated into the care givers plan as per the individual needs
- Implement wrap-around support, which is led by the client, monitoring and reviewing them regularly
- Put in place risk assessments and safety strategies prior to outreach support and home visits.
- Work in a Trauma Informed Care framework.
- Provide Homelessness industry specific support to local Salvation Army services.

What you will bring
- Tertiary or other qualifications within community services, social work or related discipline (Minimum Diploma Level)
- Demonstrated experience providing case management and creating case plans for disadvantaged and/or complex needs clients.
- A robust understanding of Trauma-Informed Practice
- Awareness of mandatory reporting guidelines is highly desirable
- Solid level of experience (minimum 2+ years) in the community service industry.
- Experience working in the Homelessness field in the Hunter region will be highly regarded.
- Experience of working with the inclusive practice, including with people from culturally and linguistically diverse backgrounds is highly desirable
- A valid Employee Working with Children Check
- A valid Driver’s licence
- A willingness to undergo a National Police Check
