Why this role matters

The Salvation Army Australia’s Homelessness Stream provides high quality, person-centred services to people at risk, and or experiencing homelessness. We support all cohorts in case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role

We are seeking a value driven Homelessness Case Manager that will provide all aspects of outreach case management support to all cohorts experiencing or at risk of homelessness. Supporting individuals to achieve housing stability, improved social inclusion, connection to support systems and the wider community.

You will successfully
- Role is primarily focused on providing outreach case management support to people experiencing, or at risk of, homelessness.
- Case plans appropriate to individuals' circumstances are to be developed and updated alongside the individual and tailored according to their goals, background and needs.
- Case plans for dependants to be developed individually or incorporated into the care givers plan as per the individual need.
- Risk assessments and Care Plans are to be put in place and responsive to support individual’s needs.
- Client information, reports and data collection should be entered and recorded into the relevant database, consistently and in accordance with TSA, and SHS procedures, policies and legislation.
- Evidence of working effectively with internal/external stakeholders and related agencies.
- Responsible for staying up to date with all internal training.
- Contribute to a workplace environment which supports peers, develops teamwork and ensures the provision of quality services for TSA clients.
- Participate and attend internal and external supervision and team meetings, 80% attendance is required

This is a part time, 15.2 hours, fixed term 2 years, based in Hamilton, NSW.

Salary and conditions are in accordance with the SCHADS CRISIS 2

What you have
- Tertiary or other qualifications within community services, social work or related discipline (Minimum Diploma Level, working toward a degree)
- Understanding of Trauma Informed Practice:

- Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients.
- Experience of working with an inclusive practice, including with people from culturally and linguistically diverse backgrounds, LGBTQIA+, women, men & families is highly desirable.
- Experience working in collaboration with other organisations to develop and tailor programs to meet clients’ needs.
- Awareness of mandatory reporting guidelines is highly desirable
- Current paid Working with Children Check & National Criminal History Check.
- Current Driver’s license is required.
