**Crescent Respite is seeking a skilled and experienced Disability Team Leader, for SIL accommodations around the Western Suburbs of Melbourne. Pay is at Level 3.1 as per SCHADS award.**

**About the role**:
We're looking for a passionate leader with experience in SIL & Respite care.

This full-time role offers a dynamic opportunity to lead our team of residential support workers while ensuring our various accommodations are managed optimally to provide unwavering support to our clients. Notably, it includes travel, allowing you to personally oversee and enhance our services across the western suburbs, making a direct impact on the well-being of those we serve.

**Tasks and Responsibilities**
- Collaboratively establishing client care plans and overseeing all care provided from Support Workers
- Responsible for the rostering, supervision and training of all direct support staff within your houses including monitoring of absenteeism and recommendations for escalation.
- Maintain and build relationships with existing Participants and key referrers such as Support Coordinators, Plan Managers, and Allied Health Professionals.
- Reporting incidents immediately in line with the NDIS Incident reporting.
- Providing direct support, supervision and guidance to participants to implement their personal goals and NDIS plan.
- Assisting with appointments, follow up activities and plans with Rostering Officer
- Prepare and collate reports as required by stakeholders
- Assisting in the onboarding of new participants into the Organisation.
- Management of accommodation budget and expenditure.

**Selection Criteria**:

- At least 12 months of experience in a similar role, preferably in the Disability or Community services sector
- Certificate III or above in Disability, Community Services or equivalent
- Passionate to work alongside clients with disability and a customer service focused approach
- Experience working with rostering systems and Microsoft 365 suite.
- Proficient written and verbal communication skills
- Willingness to travel to the SIL houses and Respite Facility - across Western Suburbs

**Mandatory Compliance Requirements**:

- NDIS Worker Screening Check
- Valid Victorian Working with Children Check
- Current First aid and CPR certificate
- Current Victorian Driver’s License
