**What You Will Be Doing**
Peninsula Health Care Finder is a newly funded program that seeks to support vulnerable clients over the age of 50 who are at risk of homelessness, with housing support and service system navigation. The position provides specialist and intensive case management to help people understand and access housing options and aged care services and connect with other relevant supports in the community. You would be required to support a caseload of approximately 20 clients.
**What You Need**
We anticipate you will have:
- A qualification in Social Work, Occupational Therapy or Nursing (RN/EN) or Community Development Work and hold a relevant registration with your governing body
- Experience in working with clients with complex needs
- Experience working in health care, community health or outreach settings
- An applied understanding of the issues and implications of homelessness for clients with complex health and social needs
- Well-developed organisation, time management, planning and priority setting skills
- Highly developed communication and documentation skills
- Excellent IT, computer systems and Microsoft Office skills and the ability to competently learn and process new information and systems
- Demonstrated commitment to the Social Model of Health and principles of trauma informed practice
- Current driver’s licence and willingness to transport clients where appropriate
