Little bit about the role

As a Community Connections Case Manager, you’ll provide intensive, person-centred support to adults under 65 who are homeless or at risk of homelessness across the Hume and Merri-bek regions. Including those sleeping rough with complex, unmet needs.

Based at Ozanam House in North Melbourne, this role delivers short to medium-term case management through an outreach model, helping clients address both their housing and support needs to sustain long-term accommodation.

You’ll work collaboratively within a multidisciplinary team to create holistic, goal-oriented case plans, connect clients to housing, health and community services and advocate for positive, sustainable outcomes.

The role requires strong coordination, assertive outreach, and the ability to manage competing priorities in a high-volume, complex environment. All while upholding VincentCare’s values of respect, inclusion, and recovery-focused practice.

Key Selection Criteria

- Demonstrated experience working with people who are homeless or at risk of homelessness.
- Strong understanding of the housing service system, including the Opening Doors Framework and housing options.
- Ability to assess and manage risk, conduct assertive outreach, and build trusting relationships with clients.
- Knowledge of intersecting service systems (mental health, AOD, NDIS, justice, and family violence).
- Excellent communication, problem-solving, and stakeholder engagement skills.
- Proven ability to maintain accurate case notes, records, and compliance with program and legislative requirements.
- Relevant tertiary qualification in social work, community services, or related discipline (minimum 1–2 years’ experience desirable).
- A valid Victorian Driver’s Licence.
