Community Connections Australia (CCA) is a NDIS and iCare accredited service provider that provides high quality and personalised supports for people living with a disability throughout the Sydney metropolitan area. CCA is looking for compassionate and positive can-do person to join our team as a Service Coordinator.

This role ensures that the people who receive services from CCA are placed at the centre of their chosen service delivery. They assist the people we support to
- realises and attains their goals,
- receive high quality support and:

- ensure that they are satisfied with their services from CCA.

As a member of team you will play an invaluable part in upholding our vision of supporting people to live independently in their own home, in the community of their choice and to receive the services they choose.

Essential requirements include:

- Relevant Tertiary Qualifications
- Professional experience in client facing service delivery
- High level of verbal and written communications skills
- Attention to detail and documentation
- Ability to self manage and prioritise complex case load
- COVID Vaccination Certificate
- Willingness to obtain a NDIS Worker Screening Check on commencement with CCA
- NSW Drivers Licence and a reliable and insured vehicle

Desirable requirements:

- Experience in and/or understanding of the NDIS sector

**Job Types**: Full-time, Permanent

**Salary**: $82,000.00 - $88,000.00 per year
