- Jewish Care Victoria, a not-for-profit organisation, is the largest provider of aged and community services for the Jewish Community in Victoria. Services include Disability, Mental Health, Youth and Family Services, Aged Residential Care, Assessment Referral and Individual Support Services, Community Aged Care and Holocaust Survivors Programs.**The role**
- As a Support Coordinator for our home care packages program, you will provide advice guidance and implementation of in-home supports to help Elders live comfortably and happily in their own homes.- This role is of the greatest importance and will require you to take into account the Elder's support needs, aspirations, choices and values, ensuring that the client's financial profile allows for the supports requested. It will allow you to use your well developed skills in problem solving and network building, and you will be supported by a knowledgable, warm and well established team of likeminded professionals. The care provided to Elders living in their own homes is some of our most important work, especially as we emerge from what has been a trying time for those who may have been feeling isolated.**Some of the tasks that will be involved in this role will be**:

- Identify needs based on an assessment of the Elders’ functional capacity, via discussion with them, their family or support person.
- Visit Elders in their homes in order to undertake assessments reviews and maintain contact with them and their families.
- Develop Support Plans in collaboration with Elders that meet their goals.
- Develop and maintain a budget in collaboration with your client in accordance with the Australian Government Guidance for Home Care Packages.
- Use of interpreters either by phone or in person when required.
- Fulfil reporting and compliance requirements within required timeframes.
- Understanding of the health and welfare issues faced by older Australians wishing to remain in their homes.
- Experience in undertaking complex in home assessments, implementing Support Plans and managing budgets.
- Evidence of fluent written and oral communication skills.
- Demonstrate compassion and empathy when working with Elders, including ethno-specific groups and Elders affected by trauma such as Holocaust Survivors.
- Demonstrate technological competency skills necessary to perform job skills (i.e. Microsoft Office suite and client management systems)
- Tertiary Qualifications in Human Services, Nursing, Allied Health, Health Management or similar.
- Ability to work autonomously, prioritise and multitask.
- Ability to speak Russian will be well regarded but not essential.
- Case Management experience is desirable.
