**Position Summary**
- Improve the patient experience by providing care in the comfort of their own home
- Assist patients to achieve their care goals and best possible level of health and function
- Promote and support safe and early discharge from hospital settings
The successful applicant will work in partnership with a strongly committed team to provide high quality and efficient care and support to patients in the home environment.
- Possess strong interpersonal skills and work well in a team environment
- Have experience working with older adults in the community and/or hospital environment
- Have good time management skills and demonstrate a flexible and adaptable approach
- Are committed to providing safe and high quality care as instructed by allied health professionals
**To be successful in this role you must have**:
- Allied Health Assistant Certificate III or IV or other appropriate qualification
- Previous experience working in hospital or community settings
- Computer literacy
- A current Victorian drivers licence
