**About the Role**

**Early Supported Discharge (ESD)**
- The ESD program is a ‘rehabilitation in the home’ program offered to patients with neurological conditions that would benefit from a high intensity multidisciplinary team input
- There are currently 10 virtual beds with an aim of expanding to 20 beds
- It aims to allow earlier discharge home by providing inpatient level care and allied health input whilst being in the comfort and safety of a patient’s own environment
- It is a time limited, goal directed program lasting from 2-4 weeks

**GEM at Home**
- Aiming to enable earlier discharge of patients over the age of 65 whose family and/or carers can support them at home whilst they continue to make functional and cognitive improvements following hospital admission
- Provides comprehensive Geriatric care and assessments from the patient’s home. Currently referred by Western Health acute and subacute wards via SNAP (Subacute and Non-acute Assessment and Pathways)

Approximately 15 -20 (flexible) virtual beds based in the community with a length of stay of approximately two weeks dependant on clinical need. This role includes community home visits and assessments, Telehealth, exposure to rehab clinics, exposure to stroke rehabilitation and community discharge planning options in the elderly. It also includes participating in team meetings, family meetings, communicating management and discharge plans to patients and their carers/families and other involved health professionals. Roles also involve completing discharge summaries and scripts.

Western Health offers a supported training environment where Senior Medical Staff make an effort to know you personally. Junior doctors in the General Training Stream are supported by a dedicated Director of Clinical Training and Medical Education Officer.

We have a strong focus on junior doctor education with a dedicated weekly intern and HMO education program, simulation and workshops for junior doctors, hospital grand rounds and a broad range of unit-specific education sessions. The Western Centre for Health Research and Education (WCHRE) at the Sunshine Hospital campus is a purpose-built teaching and research facility, including a state-of-the-art simulation centre.
